Having same problem with a Java application:
Jfritz version 0.7.3.10 (http://www.jfritz.org) running on vnc server machine.
With video hook driver 1.00.22 activated jfritz on vnc client only shows titlebar, minimize-, maximize- and close button. Inside the window you only can see the background, so window content never seems to be updated.
Have checked "Enable capture alpha blending" and tried many combinations of settings either on vnc server or on the client, but only disabling video hook driver is showing correct content of jfritz's window.
Using ultravnc server and viewer 1.0.8.2 both on Windows XP SP3 32bit. Using connection with rc4 key-file with Sean E. Covel's MSRC4Plugin.dsm version 1.2.4.0. Using Java Build 1.6.0_18-b07, but was same error with 1.6.0_17 and older versions.

can you go back to older driver of your nvidia
latest driver nvidia for xp is not well optimized. sometime older driver is better.

This problem exists since a long time.
I remember this problem with Nvidia's driver since early 160ies or even older ones. Since then I tried with many versions (16x.x, 17x.x and 18x.x), so I don't think, this could be a reason.
This problem also arrived with Ultravnc 1.5.x and 1.6.x.

could you close winvnc or stop uvnc_service
rename ultravnc.ini to ultravncold.ini
start winvnc as application
winvnc admin properties would show to ask you 2 password
set it and enable capture alpha-blending and any setting
OK close winvnc
this create a new ultravnc.ini
start uvnc_service if run as service
or start winvnc
connect vncviewer to your winvnc and look if something change.
if we can't reproduce, no luck for have a fix.

Tried uninstalling UltraVNC, cleaning UltraVNC directory and performing a clean install from scratch with no success.
Did anyone yet try to reproduce with a Java application such as JDownloader (http://jdownloader.org/) or JFritz (http://www.jfritz.org) running on UltraVNC server?
Hint: UltraVNC server is running as a service and Java applications are running as a user with administrative rights.

i have same problems in diferents machines, with java and DirectX/OpenGL games. I tried ultravnc server from v1.5x to v1.8.0.2 without results. Of course, I have administrative rights. I have nvidia cards (gf6150 and gf9400) and ati cards (1900xt, hd5770), Windows XP sp3 in all machines and the lastest drivers.

I have 3 Vnc servers,
1: Win 2000, single screen: Java works OK with or without mirror driver
2. Win XP, dual screen: Java works NOT OK with mirror driver. Without mirror driver Java works OK
3. Vista, single screen: Java works OK with or without mirror driver
All above are 32 bit systems.
Vnc Server versions can be seen below.

I still investigate and all machines with this problem have an ati vga. I installed the latest drivers and directx redist but the bug remains.
When I connect to the remote machine the directdraw's option in dxdiag is enable but locked. If I disable directdraw before connected the java's control panel shows correctly.

All my machines with ths problem have a nvidia vga (geforce 9400 and geforce 6150). I have Direct Draw's and Memory acceleration optons in dxdiag "not avaible".
