Customizing Pchelpware - how do you do it?

I've read about customization on http://www.uvnc.com/pchelpware/customization.html and have created a customization file and called it "custom.txt". What do I do next? There's nothing about what do do with the custom file on the http://www.uvnc.com/pchelpware/customization.html page.

I see that this is two years old and the patch is still not included in the download package on the download page. Still two separate zip's. Does this mean that the devs dropped pchw altogether and forgot about it? Or is it because there's a reason you would want the unpatched version?
