It also seems (from the MSDN thread) that the stub launcher API call will fail if no-one is logged on, so it will be impossible to serve the initial logon screen.

I use UltraVNC server and viewer from BartPE bootable CD. There is a standard plugin for UltraVNC. Google for BartPE. It's free (need Windows license for the PC you boot the CD from).

Sorry, but it is not clear what you want to accomplish.
VNC is typically used to access the console, and it works as long as the console is also session 0.

Try using ZoomIt from http://sysinternals.com on the clients' screen
(I suppose that means the PC running VNC server).
It has a temporary "draw" mode that works fine.

It has long since been inherent in the RFB protocol that framebuffer updates are only sent upon receiving a update request; it is part of the "automatic" adaptation to the transfer speed. And it works just fine for slow bandwidth with low latency.
